  1. Fill Out the PS Form 1583 and have it notarized. Upload two forms of ID:

    • Two forms of ID DOCUMENTS NEEDED TO UPLOAD WITH YOUR PS1583 form are:

        • Second Form of ID is a Address verification one of these: Home or Vehicle Insurance, Current Lease, Nondrivers's ID Card, Mortgage or Deed of Trust, Voter Card, Car registration, Birth Certificates, Social Security Card.

  • Alternatively if  you need keys mailed, email a forwarding address, and we will send your keys and postage fees for your package.

 

  • No forwarding fee for the first two pieces of mail forwarded to you.
