You will need two acceptable forms of identification for your mailbox registration.

Address Verification
An acceptable document showing your address may include:

Birth certificates, Social Security cards, and credit cards are NOT accepted as substitutes for the required USPS identification documents.
Please submit clear copies of the required documents along with your completed PS Form 1583.
